CANCELLATION: Notice of cancellation must be given 24 hours in advance or patient will be responsible for a cancellation fee equal to $50 per patient for follow-up visit or $125 per patient for initial visit.

INITIAL VISIT: When scheduling initial consultation and treatment, patient will be required to provide credit card information to secure the appointment.

PAYMENT: Payment is required at the time services are rendered. Cash or Check is preferred. MasterCard, Visa, American Express accepted.
